Individualized Supports and Services
Just as an item of clothing marked “one size fits all” cannot truly fit everyone well, it’s not possible to design one service that will meet everyone’s needs. Simply put, individuals need individual plans to meet their needs. With this approach, Thresholds consumers take an active role in designing their individual Person Centered Plan for service. Consumers are encouraged to include family, friends and other supports in the development of their plan.
Services include:
- Supports Coordination
- Adult Respite Services
- Consultation to nursing homes
- Coordination with Adult Foster Care Homes
- Representative Payee services for persons-served
