Residential Services Program
The Residential Services Program provides residential services to persons with a developmental disability who are at least 18 years of age or older. The Program emphasizes a “home-like” atmosphere that feature an environment of stability and growth for the persons living there. The cost to live in one of the programs is based on “ability to pay” with the SSI rate for a particular program being the minimum.
We provide residential services to persons with medical, behavioral, safety, and skill enhancement needs. We use the Person-Centered Planning approach to not only determine the needs, but also to plan for them. This is all accomplished by staff consisting of: social workers, registered nurses, registered dietician, administrators, and support staff ( secretary, psychiatrists of Thresholds, and staff from our Fiscal Deptartment). These staff persons provide the following services:
1. Supports Coordination Services
2. Nursing Services
3. Nutritional Consultation
4. Enhanced Health Care
5. Social and Recreational Opportunities
5. Individualized Support and Training
6. Social and Recreational Opportunities
7. Family Participation
8. Transportation (for some programs)
Our Residential Services Program includes a variety of residential programs and options. All programs comply with Federal policy, which prohibits discrimination based on race, color, sex, age, handicap, or national origin.
